Fulfilled Lives service Case Study 1

The Fulfilled Lives project supports those living with dementia and cognitive impairment to maintain their independence and stay connected to their communities. The aim is to put people at the centre of provision and to make sure that support is not only tailored to the individual but utilises the assets of the community and wider support networks for a holistic support approach.

Traditional services often tend to focus on physical care rather than emotional, social or economic wellbeing - this project embeds prevention at its core and promotes independent living though a structured and evidence-based approach.

Raymond’s story…

For 81-year-old avid walker *Raymond, of Ammanford, it’s essential he’s able to continue living independently to enjoy his daily exercise. Although supported by his daughter and friends,  he lives alone in a complex flat and often forgot to take his medication. He relied upon his family and friends to remind him when he needed to take them each day, as well as cook his meals.

Due to his vascular dementia, Raymond’s family were concerned he may go walking during the evening as he had tried to leave his flat previously late at night. This became a great worry.

However, to support his safety, a door alarm has been installed at his flat and if, at any point this is triggered, it will alert the Complex’s on-site carer who would be able to assist and ensure Raymond is safe, providing his family with peace of mind 24/7.

A Canary monitoring system was also installed in Raymond’s flat to help provide an idea of his typical day-to-day routine to ensure he was receiving all the necessary help to remain living independently for as long as possible. The system showed he would typically go to bed around 10pm and get up around 5.30am each day. The system was consistent during the entire time it was installed. Once his routine was suitably established the monitoring system could be removed.

To allow Raymond to continue his loveable walks safely, he has been provided with an Oysta Pearl+, which includes a GPRS-tracker system that links to his son-in-law’s phone. So, if he went out walking and suffered an unexpected fall, or got confused as to where he was, Raymond could simply press the emergency button on the unit, or the automatic fall function would immediately activate, alerting his son-in-law that he required help.

Raymond also utilises a medication dispenser and tipper to make sure taking his tablets independently is made as easy as possible for him.
