Delta CONNECT wins digital transformation award in the MJ Awards

Delta Wellbeing has achieved the Digital Transformation award in this year’s MJ Awards for the Delta CONNECT project.

The MJ Awards acknowledges the remarkable effort of those in local government dedicating themselves to supporting local communities and showing perseverance in the face of challenges.

The MJ Awards winner brochure said: “In an exceptionally strong field, the judges were impressed with not only the ambition and scale of Delta CONNECT, the council's health and social care digital transformation programme but also the passion and commitment of those who have delivered it - and continue to do so. Judges called it 'truly impressive.'”

Delta CONNECT is a wraparound 24/7 support service available across Carmarthenshire, Pembrokeshire, and Ceredigion.

The service includes a wellbeing assessment, pro-active wellbeing calls, digital support, help to re-engage with the community and access to a 24/7 community response service.

Delta Wellbeing’s bespoke Technology Enabled Care (TEC) packages include devices that enables people to get help at the touch of a button, and specialised equipment tailored to the customer’s needs. This can include GPS tracking watches, epilepsy sensors, medication dispensers, door sensors, bed sensors, digital tablets, and more. The service can provide a digital solution to almost any condition or need.

The project currently has supported over 5000 individuals, and currently offers a free trial until March 31, 2023. With an 80% retention rate after the free period, CONNECT has succeeded in supporting clients with their independence, while ensuring they stay safe in their own homes for longer.

The project is funded by the Welsh Government’s Transformation Fund, enabling the West Wales Care Partnership Board. The board brings together Carmarthenshire, Ceredigion and Pembrokeshire County Council’s, Hywel Dda University Health Board and representatives of the third and independent sector. This partnership helps shape the future of health and social care services across west Wales.
