Silver innovation award for Delta CONNECT service

Delta Wellbeing has won silver in the innovation category at the 2021 iESE Public Sector Transformation Awards. 

The award was for developing and bringing to market new innovative technologies and facilities through Delta CONNECT, which has been transforming the delivery of Delta Wellbeing’s local public service. 

Carmarthenshire County Council’s Cabinet Member for Health and Social Care Cllr Jane Tremlett said: “We are absolutely delighted that the Delta CONNECT service has gained recognition at this year’s awards. 

"As a council, we recognised the value of TEC a few years ago and its potential to support the challenges our health and social care systems were facing, and so prioritised the transformation of the service. More recently it has been at the heart of our COVID response, enabling us to protect the 8,000 people shielding with pro-active calling, food, medicine and practical, triaged support.” 

The Delta CONNECT wrap-around support service is available in Carmarthenshire, Ceredigion and Pembrokeshire. It includes wellbeing calls, Technology Enabled Care (TEC) packages tailored to individuals’ needs, access to an appropriate 24/7 Community Response Team and digital support to help loved ones stay connected. 

The pilot project has been funded by the Welsh Government’s Transformation Fund, enabling the West Wales Care Partnership Board - which brings together Carmarthenshire, Ceredigion and Pembrokeshire County Council’s, Hywel Dda University Health Board and representatives of the third and independent sector - to work together to help shape the future of health and social care services across west Wales.
