Are you passionate about supporting vulnerable individuals and promoting independence?

In this vital role, you will be the single point of contact providing advice, information, and assistance for all social care and health service needs, based in various NHS buildings within Carmarthenshire. You'll work closely with multi-disciplinary hospital teams to ensure the best outcomes for clients through assessments, planning safe discharges, and connecting them with community preventative services.

As a Community Wellbeing Officer, you’ll use your communication skills to gather information, evaluate risks and eligibility, and offer tailored advice over the phone, online or in-person hospital visits. You'll empower vulnerable adults and their carers by giving them a voice in their care, promoting independence and social integration. Your outstanding customer service skills will be invaluable in providing prompt, courteous and accurate responses during crisis situations.
