🚑 24/7 community response team that supports non-medical emergencies such as non-injurious falls and welfare needs.

🫶🏼 Wellbeing support activities to encourage participation in activities that are important to your wellbeing.

Thanks to funding from the Welsh Government’s Health and Social Care Regional Integration Fund (RIF), the Delta CONNECT service is heavily subsidised, making it more affordable for everyone to access.
