Eileen Rees, aged 87, has multiple health conditions, including osteoporosis, diabetes, and a history of fractures, and lives alone, but remains determinedly independent. After a fall at home that resulted in a fractured pelvis, she was admitted to Glangwili Hospital. Her desire to return home to her beloved cat and resume activities like shopping and attending her local community centre was strong, but she recognised the need for additional support with personal care and meals.

Delta Wellbeing played a pivotal role in facilitating her safe discharge and recovery providing emergency bridging support while her reablement package was being arranged, delivering personal care and meal assistance three times a day. This immediate intervention allowed Mrs Rees to return home safely and begin regaining her independence.

She was also given a lifeline, so that if there were any concerns or issues, she could press the red button day and night to get through to someone straight away. The use of Technology Enabled Care gave her reassurance, knowing that help was always within reach in case of another fall or emergency. 

The response team also established a strong rapport with Mrs Rees, offering not just practical assistance but emotional reassurance that boosted her confidence.
