Delta Wellbeing highly commended at Social Care Wales Accolades for outstanding partnership work

Delta Wellbeing, in collaboration with Carmarthenshire County Council and Hywel Dda University Health Board, has been highly commended at the Social Care Wales Accolades 2025 for its pioneering Delta CONNECT service.

Delta CONNECT was one of three finalists in the highly competitive Working in Partnership category, sponsored by Life Sciences Hub Wales.

The Accolades shine a spotlight on the exceptional work in social care, early years and childcare across Wales and highlight teams and organisations making a real difference to people’s lives through innovation, compassion and collaboration.

The judges praised Delta CONNECT for its innovative, preventative approach to care, delivered through strong multi-agency partnerships. The judges were impressed by the multi-disciplinary team working and collaboration with the ambulance service, noting the preventative aspects and far-reaching impact of the work.

Funded by the Welsh Government’s Health and Social Care Regional Integration Fund, through the West Wales Regional Partnership Board, CONNECT focuses on delivering proactive care through Technology Enabled Care (TEC) and personalised support, aimed at older adults, those with chronic conditions, and individuals at risk of social isolation. 

The project offers significant benefits by improving independence and wellbeing through personalised care at home. A core part of the service includes proactive wellbeing calls and a 24/7 community response team, ensuring continuous monitoring and swift intervention in a crisis.

Together with Delta Wellbeing’s hospital-based Blue Army staff, these services help people stay safe, independent and connected in their own homes for longer – easing pressure on hospitals and social care services.

Carmarthenshire County Council’s Cabinet Member for Health and Social Services Councillor Jane Tremlett said: “Delta CONNECT is a brilliant example of how working together across health and social care can make a real difference. This recognition reflects the hard work and dedication of our team and our partners – who go above and beyond every day to support the people of west Wales.

“The success of Delta CONNECT highlights the importance of collaborative efforts in delivering high-quality care and support to our community; improving lives, giving families peace of mind, and helping people live well at home for longer.”

While the overall winner in the category was Connecting Carmarthenshire, the judges were clear on how close the decision was with just a few points in it, praising all three finalists for their impact and innovation.

Delta Wellbeing is a Local Authority Trading Company owned by Carmarthenshire County Council, specialising in assistive technology, proactive monitoring and response services to support individuals to live independently and safely at home, reducing reliance on acute and long-term care.
