Wellbeing calls are making a difference to residents across west Wales

Llesiant Delta Wellbeing has carried out more than 80,000, pro-active calls to CONNECT clients since the service was launched.

The team of friendly advisors regularly check up on their clients, calling them for a chat or just to wish them a happy birthday! They are on hand 24/7 to give families peace of mind all year round.

Delta CONNECT is an enhanced lifeline and telecare service which is available in Carmarthenshire, Ceredigion and Pembrokeshire.

As part of the client’s initial wellbeing assessment, the team decides on a bespoke wrap-around support package. The pro-active calls could be weekly, monthly, or as often as they need.

It supports the project’s aim to combat loneliness in older and vulnerable people, and also to ensure that their needs are being met.

Figures show that as of the end of April 2023, a total of 83,317 pro-active calls have been made to clients with 80% having improved or maintained their overall wellbeing.

“I’m very appreciative of the support Delta offers,” said a CONNECT client. “I’ve had wellbeing calls from Carys and Joshua recently and it’s been lovely to speak with them and Rebecca. I believe Delta staff genuinely care and I take this away from my calls every time.”

Another client said: “Louise called me weekly after my cancer operation and it kept me going when things were really bad. I looked forward and appreciated her calls so much. I’m so grateful for your care and the conversations we had. Thanks Delta for all the support.”

“The friendly calls I receive have been delightful and always pleasant and not hurried in any way,” added another. “Most appreciated!”

As part of CONNECT, clients receive a technology enabled care package, depending on their needs, to support their independence. It includes a lifeline and pendant which links through to the monitoring centre, providing 24-hour support at the touch of a button.

They also have access to a 24/7 response team who will attend the client’s home when in crisis should they ever need it.

Delta Wellbeing’s Strategic Innovations Programme Lead Carla Dix said: “Clients have an in-depth assessment taking that focuses on what matters to them. A wellbeing plan is developed together, identifying any pro-active support that can be provided as well as pathways that can assist with loneliness, falls and carer stress for example.

“These plans are monitored through our pro-active wellbeing calls and if any issues or triggers for crisis are identified we can then take action through practical support.”
