Technology Enabled Care

Telehealth is technology to help individuals monitor their health from the convenience of their homes. It adds to the care already offered by GPs/nurses and specialists.

Patients will be provided with equipment such as electronic sensors which will monitor levels of activity, blood pressure and oxygen readings. The readings will automatically be submitted to their health professional through an APP on a mobile phone or tablet.

Telehealth is one technology which is being rolled out across the Hywel Dda UHB region. Along with video consultations, text messages, questionnaires etc, telehealth provides individuals with all they need to manage their own health.

To assist in preventing falls and identify other risks, we use a telehealth wearable device to collect information associated with frailty and the risk of falling.

One of the devices to help us identify these risks is a simple watch which monitors an individual’s daytime activity, sleep pattern and mobility to see if they need any additional support.

A wearable device is one technology which is being rolled out across the Hywel Dda UHB region. Along with video consultations, text messages, questionnaires etc, telehealth will provide individuals with all they need to manage their own health. 

Telehealth will become a common feature of care and support to patients across west Wales. It will give individuals and healthcare professionals a number of benefits, such as:

  • The ability to pick up any health issues at the earliest opportunity
  • Reducing the need to attend outpatient appointments
  • Providing regular communication with health professionals via text messaging and video consultation
