Support for carers

Whether you are a friend, family member or a carer concerned about the wellbeing of someone, we can give you the help and expert advice that you may need to support the safety and wellbeing of the person you are caring for.

Many products can be linked to a person rather than our monitoring centre so that an alert can be raised to someone close at hand, you can choose which option suits your needs.

When you contact us to enquire about services you will speak to a professionally qualified advisor who will provide information, advice and assistance on matters relating to health, social care and wellbeing. Our team will ask you what’s important to the individual you are caring for and to you, this will help us understand the situation and put together a bespoke package of practical advice and technology that will best meet your needs. Our key aim is to help people live as independently as possible for as long as possible.

Carers Emergency Card

Carers ID / Emergency Card

We provide a Carers ID Card / Carers Emergency Card to unpaid carers in Carmarthenshire, on behalf of Carmarthenshire County Council. It is a wallet-sized identification card which can be used for:

  1. Access to discounts:  Carers can access available discounts. They will also be offered a carers assessment, either on their own or have a joint assessment with the ‘cared for’.
  2. Emergency response: Carers can carry these cards and if they are involved in an accident or are unable to return home to maintain their caring responsibilities, emergency services or others will be able to make contact with relevant people to let them know that there is someone at home who cannot manage without help. (This is optional)

The card is valid for 3 years and you can reapply when it expires if you are still an unpaid carer.

What kind of discounts can I receive?

Local businesses in Carmarthenshire offer discounts to cardholders. Participating businesses display a sticker at their premises.

How will the carers emergency element work?

This ensures that in an emergency, relevant contacts are notified to provide support to the person you care for.

Complete the registration form giving details of the person you care for, and up to three nominated people who can respond in an emergency or other unforeseen circumstances.

We will then process the form and send you the card, which has its own unique ID number. In the event of an unexpected incident or emergency, the card will have our emergency number it, and the person calling can quote the unique ID number to inform our advisers so they can activate support. We will call the nominated person(s) to provide interim unpaid care support to avoid the risk of deterioration or crisis to the vulnerable person at home.

What if I don't have anyone who could temporarily take my place?

You can still register for the scheme. In the event of an emergency, we will automatically call social services, who will respond and, if necessary, make alternative arrangements.

What does the nominated person(s) have to do?

The nominated person should know the person you care for and the support they need. We record their details so they can be contacted in an emergency if you are unable to provide care. It is best if they are aware of this and agree to be contacted. They should know how to access your home and what to do in such circumstances.

Nominees should also be aware of which GP the person you care for is registered with, and of any other family member(s) they should contact if they are called upon.

How do I apply for a card?

You can apply quickly and easily by completing our online registration form, which will be sent directly to our team. Please make sure you have the permission of the person you are caring for and the nominated person. If you prefer, you can download the PDF document instead to print off and fill in, and return to us in the post at Delta Wellbeing, Eastgate, Llanelli, Carmarthenshire, SA15 3YF

What if my circumstances change?

Please contact us on 0300 333 2222 to report any changes.
